It would benefit customers greatly to have the ability to create custom commands. There are instances in which conforming to hospital tradition outweighs training staff to conform to Vocera commands. For Example: the administrator can create a custom command called 'Beam Me Up' that, when said initiates an urgent broadcast to the security team. The possibilties are quite endless, but you can see how this offers the ability for Vocera to adapt to a customers environment as opposed to the other way around.
